You can obtain a copy of your medical records from The Medical Center of Aurora and Centennial Medical Plaza through our Health Information Management office. Please note that birth and death certificates are processed through the Colorado Department of Health Vital Statistics Department.

Records for physician office visits will be handled urgently to ensure continuity of care.
Birth and Death Certificates
Certified birth and death certificates are processed and purchased thru The Colorado Department of Health, Vital Statistics department.
